ODV Guide℠ — Transitions of Care

Post discharge support that stabilizes patients and keeps them connected to primary and specialty care. EMTs and Paramedics assess on scene, NPs finalize the plan via telehealth, and our team schedules follow ups while addressing social needs.

ODV Pathways℠ — Treat-in-Place / 911 Navigation

For low acuity calls, EMTs and Paramedics assess and treat in place, connect to a same day NP telehealth visit, and divert unnecessary transports while ensuring clinical safety.

Proactive outreach for frequent utilizers with immediate intervention, care management, and connection to community resources to sustain engagement and improve outcomes.

How it Works

  1. Triage & dispatch from 911 partners, hospitals, or health plans
  2. On-scene assessment by EMT/Paramedic (vitals, safety, education)
  3. NP telehealth same day to confirm diagnosis, prescribe when appropriate, and set the plan
  4. Follow-through with scheduled appointments, referrals, and ongoing checks
